Atvērts arteriāls vads (PDA) ir viena no biežākajām sirds patoloģijām (5-10 % no visām iedzimtām sirdskaitēm). Visbiežāk šī patoloģija tiek diagnosticēta jaundzimušajiem, tomēr tā var tikt atklāta līdz pirmsskolas vecumam vai pat pieaugušo vecumā. Ja neliels PDA nerada smagus simptomus, tad šajā gadījumā ārstēšana var nebūt nepieciešama. Lielākus, un/ vai hemodinamiski nozīmīgus PDA nepieciešams ārstēt – sāk ar medikamentozu terapiju (tikai priekšlaicīgi dzimušiem), ja nelīdz, tad ķirurģiski (caur endovazālu pieeju vai konvencionāli, veicot torokotomiju vai retākos gadījumos sternotomiju un PDA slēgšanu). Pētījuma mērķis: Veikt datu analīzi par perkutāno transvazālo un ķirurģisko PDA slēgšanu laikā dzimušiem bērniem vecumā 0-18 gadiem Latvijā laikā periodā 2018. – 2022. gadā, izmantojot BKUS ārstēto pacientu datus. Materiāli un metodes: Retrospektīvs pētījums, izmantojot Bērnu klīniskās universitātēs slimnīcās (BKUS) elektronisko sistēmu “Andromeda”, pieejamās pacientu slimības vēstures, kuri atbilst PDA diagnostikas kritērijiem, ka arī bija analizētas pacienta operācijas apraksti. Statistiskai datu analīzei tika izmantota programma “IBM SPSS v.28.0”. Rezultāti: Pētījumā tika iekļauti 70 pacienti, no kuriem 71% meitenes (n=50) un 29% zēni (n=20). Lielāku bērnu skaits ir vecuma grupā 0-3 gadi, 60% (n=42). Pacientu vidējais vecums bija 5,41 ± 4,9203, mediānas vecums atbilst 3,5. Vidējais vecums meitenēm bija 5,3 ± 5,07 gadi, zēniem 5,68 ± 4.65 gadi. Vidēja PDA izmērs ir 3,18 ± 1.168, min=1.4 mm, max=7.0 mm, prevalē divas lieluma grupas: mazs (atbilst 1,5-3 mm) un vidējais (atbilst 3-5 mm) PDA, katrā grupa bija 31 pacients. Analizējot vienu no EhoKG kritērijam PDA diagnosticēšanai, tika konstatēts, ka 52.9% pacientam (n=37) ir plaušu hiperperfūzija (LA/Ao >1.5) un 47.1% (n=33) netika konstatēta. 94% (n=66) PDA tika slēgta perkutāni transvazāli, kas ir statistiski nozīmīgi (p=0.01), ar atvērta tipa sirds operācija tika veikta 3% (n=2), 3% (n=2) operācija bija pārcelta, dēļ pacienta akūtas saslimšanas. Pēc operācijās 5,8% (n=4) rodas komplikācijas, sledzējierīces migrācija 3% (n=2), reziduāls PDA 1,4% (n=1), neliels ligatūras iekapsulēts gabals 1,4% (n=1). Secinājumi: 1.PDA patoloģija dzimuma starpā ir biežāka meitenēm nekā zēniem (p < 0.001); 2.PDA visbiežāk atklāj vecuma grupā 0 – 3 gadi – 60 % no visiem pētījuma korespondentiem; 3.PDA visbiežāk diagnosticē nejauši EhoKG skrīningā – 57 % gadījumos; 4.Nav atklāts PDA izmērs, kas būtu vairāk sastopams – visi lielumi (mazs, vidējs, liels PDA) ir vienlīdz bieži sastopami; 5.BKUS PDA perkutāna transvazāla slēgšana ir biežāka nekā PDA slēgšana torakotomijas ceļā. 94 % PDA tika slēgts perkutāni transvazāli, un 3 % to slēdza ar atvērto tipa operāciju torakotomijas ceļā; 6.Biežākā ierīce PDA transvazālas slēgšanas gadījumā ir AMPLATZER sistēma – to pielietoja 75 % ; 7.Komplikāciju biežums atbilst literatūras datumiem - 5.8% gadījumos pacientiem pēc PDA slēgšanas ir komplikācijas. Introduction: Patent ductus arteriosus (PDA) is one of the most common cardiac pathologies (5-10% of all congenital heart disease). It is usually diagnosed in newborns but can be detected before preschool age or even in adulthood. If a small PDA does not cause severe symptoms, treatment may not be needed in this case. Larger and/or hemodynamically significant PDAs must be treated - first with drug therapy (only in premature infants), and if this does not help, then surgically (via endovascular access or conventionally by thoracotomy or, in rare cases, by sternotomy and PDA closure). Aim of the study: Analysis of relevant evidence on percutaneous transvascular and surgical PDA closure in term infants aged 0-18 years in Latvia in 2018-2022, using data from patients treated at BKUS. Materials and methods: Retrospective study using the "Andromeda" system at BKUS, analysing the medical records of patients who met the diagnostic criteria for PDA and the patients' surgical reports. "IBM SPSS v.28.0" was used for statistical data analysis. Results: The study included 70 patients, 71% girls (n=50) and 29% boys (n=20). A higher number of children were in the age group 0-3 years, 60% (n=42). The mean age of the patients was 5.41 ± 4.9203, with a median age of 3.5. The mean age for girls was 5,3 ± 5,07 years, for boys 5.68 ± 4.65 years. The mean PDA size was 3.18 ± 1.168, min=1.4 mm, max=7.0 mm, two size groups prevailed: small (corresponding to 1.5-3 mm) and medium (corresponding to 3-5 mm) PDA, with 31 patients in each group. When analysing one of the echocardiographic criteria for the diagnosis of PDA, 52.9% of patients (n=37) had pulmonary hyperperfusion (LA/Ao >1.5) and 47.1% (n=33) were not found. 94% (n=66) of PDAs were closed percutaneously transvascular, which is statistically significant (p=0.01), open heart surgery was performed in 3% (n=2), 3% (n=2) had the operation postponed due to the patient's acute illness. Complications occurred after surgery in 5.8% (n=4), migration of the occluding device in 3% (n=2), residual PDA in 1.4% (n=1), small piece of encapsulated ligature in 1.4% (n=1). Conclusion: 1. PDA pathology is more common in girls than in boys (p < 0.001); 2. PDA is most frequently detected in the 0-3 years age group - 60% of all study participants; 3. PDA is most frequently diagnosed incidentally during echocardiographic screening - in 57% of cases; 4. No PDA size was found to be more common - all sizes (small, medium, large PDA) were equally common; 5. Percutaneous transvascular closure of BKUS PDA is more common than thoracotomy closure of PDA; 94% of PDAs were closed percutaneously transvascularly and 3% were closed by thoracotomy; 6. The most commonly used device for transvascular PDA closure was the AMPLATZER system, which was used in 75% of cases; 7. Complication rates were consistent with those reported in the literature, with complications occurring in 5.8% of patients after PDA closure.
