dc.description.abstract | Mērķis. Noskaidrot radioloģisko izmaiņu spektru plaušās dažāda vecuma bērniem ar COVID-19 infekciju, kuri saņēmuši ārstēšanu slimnīcā. Metodes. Tika veikta retrospektīva analīze Bērnu klīniskajā universitātes slimnīcā, Rīgā, Latvijā. No elektroniskās datu bāzes pētījumam tika atlasītas to COVID-19 pacientu vēstures, kuriem stacionāra etapā tika veikta plaušu rentgenogramma. Plaušu bojājuma pakāpes tika iedalītas 4 kategorijās: 1. - "bronhu drenāžas traucējumi" un/vai "bronhu obstrukcija"; 2. - vieglas pakāpes bojājums (aizņēma < 1/3 no plaušu apjoma); vidējas pakāpes plaušu bojājums (ap 1/3 – 2/3 daļas no plaušu apjoma); smagas pakāpes vai difūzs plaušu bojājums (> 2/3 no plaušu apjoma). Tika analizēti šādi dati: dzimums, vecums, plaušu rentgenogrammas apraksts, ārstēšanās ilgums stacionārā, biežāk sastopamie simptomi, CRO un IL-6 līmenis asinīs, leikocītu kopējais skaits, kā arī neitrofilo leikocītu un limfocītu skaits. Statistisko datu apstrāde tika veikta ar SPSS. Rezultāti. Pētījumā tika iekļauti 99 dažāda vecuma bērni ar COVID-19 infekciju: 1. vecuma grupa - no dzimšanas līdz vienam mēnesim (n=11, 11,1%), 2. grupa - no viena mēneša līdz vienam gadam (n=20, 20,2%), 3. grupa - no viena gada līdz pieciem gadiem (n=38, 38,4%), 4. grupa - no pieciem gadiem līdz 12 gadiem (n=13, 13,1%), 5. grupa - no 12 gadiem līdz 18 gadiem (n=17, 17,2%). Plaušu rentgenogramma bez patoloģiskām izmaiņām: 1. grupā n=6, (54,5%), 2. grupā n=7 (35%), 3. grupā n=10 (26,3%), 4. grupā n=6 (46,2%), 5. grupā n=7 (41,2%); kopumā n=36 (35,6% no visiem analizētajiem izmeklējumiem). Bronhu drenāžas traucējumi un/vai bronhu obstrukcija: n=32, (31,7% no visiem analizētajiem izmeklējumiem) un 19 gadījumi no tiem bija 3. vecuma grupā. Vieglas pakāpes, kur plaušu bojājums aizņēma < 1/3 no plaušu apjoma: n=19 (18,8% no visiem analizētajiem izmeklējumiem). Vidējas pakāpes: n=8 (8% no visiem analizētajiem izmeklējumiem). Izņemot 1. grupu, smagas pakāpes plaušu bojājumu konstatēja pa vienam pacientam katrā grupā. Secinājumi. Lielāko ar COVID-19 infekciju saslimušo skaitu novēroja 1 - 5 gadu vecuma grupā (n=38). Šo respondentu vidū 19 pacientiem novēroja bronhu drenāžas traucējumus un/vai bronhu obstrukcijas pazīmes. Visās vecuma grupās, pieaugot plaušu bojājuma smagumam, samazinājās saslimušo skaits. Lai izdarītu precīzākus secinājumus, ir nepieciešami turpmāki novērojumi. | |
dc.description.abstract | Aim. To clarify the spectrum of radiological changes in the lungs of children of different ages with COVID-19 infection who received treatment in the hospital. Methods. We performed a retrospective analysis at the Children's Clinical University Hospital, Riga, Latvia. From the electronic database, was selected patient histories with COVID-19, who underwent chest X-ray during the inpatient. The degrees of lung damage were divided into 4 categories: 1 - "bronchial drainage disorders" and/or "bronchial obstruction"; 2 - mild degrees (the damage occupied < 1/3 of the lung volume); medium grade (lung damage around 1/3 – 2/3 of the lung volume); severe or diffuse lung damage (> 2/3 of the lung volume). The following data were analyzed: gender, age, description of lung X-ray, duration of treatment in hospital, more frequent symptoms, CRO and IL-6 blood levels, total number of leukocytes, as well as neutrophil and lymphocyte count. Statistical data processing was performed with SPSS. Results. The study included 99 children with COVID-19 infection of different ages: age group 1 from birth to the 1st month n=11 (11,1%), group 2 from the 1st month to the 1st year n=20 (20,2%), group 3 from the 1st year to the 5th year n=38 (38,4%), group 4 from the 5th year to the 12th year n=13 (13,1%), group 5 from the 12th year to the 18th year n=17 (17,2%). Chest X-ray without pathological changes was observed: in group 1 n=6 (54,5%), in group 2 n=7 (35%), in group 3 n=10 (26,3%), in group 4 n=6 (46,2%), in group 5 n=7 (41,2%); in total n=36 (35,6% of all). Bronchial drainage disorders and/or bronchial obstruction: n=32 (31,7% of all) and 19 of them were in age group 3. Mild grades, where the lung lesion occupied < 1/3 of the lung volume: n=19 (18,8% of all). Intermediate grades: n=8 (8% of all). Except for the group 1, severe lung damage was found in one patient in each group. Conclusions. The largest number of infected with COVID-19 was observed at the age of 1-5 years (n=38). Among them, 19 patients had bronchial drainage disorders and/or signs of bronchial obstruction. In all age groups, as the severity of lung damage increased, the number of patients decreased. Further observations are needed to draw more definitive conclusions. | |
