Kombinētas terapijas efektivitāte tīklenes vēnas trombozes gadījumā

Ievads: Tīklenes vēnas tromboze ir otra biežāk sastopamā tīklenes slimība pēc diabētiskās retinopātijas, un tas ir piektais visbiežākais akluma cēlonis. TVO sastopamība palielinās līdz ar vecumu, un vairāk nekā puse no visiem gadījumiem rodas, kas vecāki par 65 gadiem. Tīklenes vēnas oklūzijas ārstēšana ietver dažādas pieejas, kuru mērķis ir uzlabot asinsrites plūsmu tīklenes vēnās, novērst vai samazināt tās komplikācijas un atjaunot redzi. Šo ārstēšanas metožu iespējams pielietot atsevišķi vai kombinācijā, atkarībā no pacienta individuālajiem faktoriem un slimības smaguma pakāpes. Šīs metodes ietver intravitreālas anti-vaskulārais endoteliālais augšanas faktors (anti-VEGF) terapijas, glikokortikoīdu injekcijas, un lāzerfotokoagulāciju. Tādēļ ir nepieciešams izvērtētu kombinētās terapijas efektivitāti. Darba mērķis: Noteikt kombinētās terapijas efektivitāti tīklenes vēnas trombozes gadījumā. Materiāli un metodes: Retrospektīvā pētījumā tika iekļauti 190 pacienti ar tīklenes vēnas trombozi, kam trīs mēnešu laikā veiktas intravitreālas bevacizumab injekcijas un kombinētā terapija. Pacienti tika iedalīti divās grupās: (94 pacienti) ar kombinētu terapiju un (96 pacienti) ar anti-VEGF terapiju. Pētījumā tika analizetas labākā koriģētā redzes asuma izmaiņas (BCVA), intraokulārais spiediens (IOS), arteriālās hipertensijas un makulas tīklenes biezums (CRT), izmantojot optiskās koherences tomogrāfijas (OKT) izmeklējumu pirms intraviterālas anti-VEGF un kombinētas ar fokāla tīklenes lāzerfotokoagulācijas kombinētas terapijas uzsākšanas un trīs mēnešus pēc veiktās terapijas. Rezultāti: Vidējais pacientu vecums anti-VEGF terapijas grupā ir 60 gadi, savukārt kombinētās terapijas ir 56 gadi. BCVA izmaiņu analīze anti-VEGF grupā rādīja statistiski ticamas izmaiņas pirms bevacizumab terapijas uzsākšanas (p<0,001) un pēc 3 mēnešiem (p<0,003). Savukārt kombinētās terapijas grupā tika izmantoti parametriskie testi, jo dati bija normāli sadalīti, un tika novērotas statistiski ticamas izmaiņas pirms un pēc terapijas uzsākšanas (p<0,001). Anti-VEGF grupā vidējais IOS pirms bevacizumab terapijas uzsākšanas bija 18,8 mmHg. Kombinētas terapijas grupā vidējais IOS pirms bevacizumab ar lāzerfotokoagulāciju terapijas uzsākšanas bija 16,98 mmHg. anti-VEGF grupā tika konstatēta negatīva korelācija starp BCVA pirms terapijas un CRT (r= -0,56). Vāja negatīva korelācija tika novērota starp BCVA pēc 3 mēnešiem un CRT anti-VEGF grupā (r=-0,20). Analizējot IOS un CRT, anti-VEGF grupā pēc 3 mēnešiem tika konstatēta pozitīva korelācija (r= 0,60). Kombinētās terapijas grupā tika novērota pozitīva korelācija starp BCVA pirms terapijas un CRT (r=0,60), kā arī starp BCVA pēc trīs mēnešiem un CRT (r=0,69). Secinājumi: Kombinētās terapijas grupā tika uzrādīti statistiski ticami rezultāti BCVA uzlabojumā. Arteriālā hipertensi palielina TVO riska varbūtību. Kombinētās terapijas grupā tika uzrādīti statistiski ticami rezultāti CRT samazināšanās. Introduction: RVO is the second most common retinal disease after diabetic retinopathy, and it ranks fifth among the leading causes of blindness. It affects over 16 million individuals, with its incidence rising with age, especially among those over 65 years old. Men have a higher risk of RVO than women. Treatment involves various approaches to enhance retinal blood flow, prevent complications, and restore vision. These methods, including intravitreal anti-VEGF therapies, glucocorticoid injections, and laser photocoagulation, can be employed alone or in combination based on individual patient factors and disease severity. Evaluating the efficacy of combined therapy is crucial. Aim: To assess the efficacy of combined therapy in retinal vein thrombosis. Materials and Methods: A retrospective cohort study involving 190 patients with retinal vein thrombosis who received intravitreal bevacizumab injections and combination therapy over a three-month period was conducted. Patients were divided into two groups: 94 patients underwent combination therapy, while 96 patients received anti-VEGF therapy alone. The study examined changes in BCVA, intraocular pressure, arterial hypertension, and CRT using optical coherence tomography (OCT) before initiating combined therapy with intravitreal anti-VEGF and focal retinal laser photocoagulation, and three months after treatment Results: In the anti-VEGF therapy group, patients averaged 60 years of age, while in the combined therapy group, the average age was 56 years. BCVA significantly improved in both groups: before anti-VEGF therapy (p < 0.001) and after 3 months (p < 0.003) in the anti-VEGF group, and before and after treatment (p < 0.001) in the combined therapy group. Before starting bevacizumab, intraocular pressure (IOP) was 18.8 mmHg in the anti-VEGF group and 16.98 mmHg in the combined therapy group. A negative correlation was found between pre-treatment BCVA and macular edema thickness (r = -0.56) in the anti-VEGF group, and a positive correlation between IOP and CRT after 3 months (r = 0.60). In the combined therapy group, positive correlations were observed between BCVA before therapy and macular edema thickness (r = 0.60), as well as after three months (r = 0.69) Conclusion: The combined therapy group demonstrated statistically significant improvements in visual acuity. Arterial hypertension increases the risk of retinal vein occlusion. In the combination therapy group, statistically significant reductions in macular edema thickness were observed.
