Krūts rekonstrukcijas ietekme uz adjuvantās terapijas uzsākšanu krūts vēža pacientēm pēc mastektomijas

Problēmas aktualitāte. Latvijā un pasaulē krūts vēzis ir visizplatītākais ļaundabīgais audzējs sievietēm. Galvenā ārstēšanas metode joprojām ir ķirurģiskā operācija, bet liela nozīme ir arī papildus terapijai tāpēc, ka ķīmijterapija un staru terapija var nozīmīgi ietekmēt attāluma ārstēšanas rezultātus. Svarīgi, lai ķirurģiskā ārstēšana neietekmē savlaicīgu adjuvantās terapijas uzsākšanu.
Pētījuma mērķis ir izvērtēt krūts rekonstrukcijas iespējamo ietekmi uz adjuvantās terapijas uzsākšanu krūts vēža pacientēm pēc mastektomijas.
Materiāli un metodes. Tika veikts retrospektīvs pētījums, kurā analizētas Latvijas Onkoloģijas centrā ārstētās 100 krūts vēža pacientes. Pētāmajā grupā bija 47 pacientes, kurām veikta mastektomija un krūts rekonstrukcija ar sekojošu adjuvanto terapiju, kontroles grupā – 53 pacientes, kurām veikta mastektomija ar sekojošu adjuvanto terapiju. Dati apkopoti un apstrādāti elektroniski, izmantojot Microsoft Excel 2007 un SPSS Statistics 20 programmas.
Rezultāti. Pētījumā iekļauto pacienšu vidējais vecums bija 42,9 ± 7,2 gadi pētāmajā grupā un 54,7 ± 11,1 gadi kontroles grupā. Visbiežāk krūts rekonstrukciju pēc mastektomijas veica jaunas sievietes vecumā no 35 līdz 44 gadiem (51,1%, 95%CI: 37,2; 64,7). Galvenokārt (83%) krūts rekonstrukcija tika veikta ar implantu/ekspanderi (95%CI: 69,7; 91,1). Salīdzinot klīniskās stadijas abās grupās, tika konstatēts, ka starp abām grupām nav atšķirība (p>0,05). Starp grupām netika novērota arī histoloģiska atšķirība (p>0,05), 82% bija vadu vēzis. Visbiežāk krūts vēzis bija zemas diferenciācijas pakāpes (49%, 95%CI: 39,0; 58,8): 44% rekonstrukciju un 53% - kontroles grupā. Salīdzinot hormonreceptoru stāvokli, starp grupām statistiski ticamas atšķirības nepastāv (p>0,05). Izvērtējot arī limfvadu invāziju, atšķirību starp grupām nenovēro (p>0,05). Vidējais hospitalizācijas ilgums bija 16,3 ± 7,0 dienas, kas starp abām grupām atšķiras par 3,14 dienām un tas ir statistiski ticami īsāks rekonstrukcijas grupā nekā kontroles grupā (p<0,05). Salīdzinot vidējo dienu skaitu no operatīvās terapijas līdz adjuvantu terapijas uzsākšanai, tika konstatēts, ka starp abām grupām nav statistiski ticamas atšķirības (p=0,072), rekonstrukciju grupā 31,1 ± 11,6 dienas un kontroles grupā 35,8 ± 13,7. Nevienai pacientei nebija atcelta adjuvantu terapija krūts rekonstrukcijas sarežģījumu dēļ. 94% pacienšu saņēma ķīmijterapiju. Piecu gadu laikā mirušas 6 pacientes, viena no rekonstrukciju grupas un piecas – no kontroles grupas. Sagaidāmā 5 gadu dzīvildze starp grupām ticami neatšķīrās (p=0,224).
Secinājumi. Krūts rekonstrukcija neaizkavē adjuvantās terapijas uzsākšanas laiku krūts vēža pacientēm pēc mastektomijas un nepasliktina ārstēšanas attāluma rezultātus. The title of the diploma paper is “Breast reconstruction impact on the initiation of adjuvant therapy of breast cancer patients after mastectomy”.
The main problem. Breast cancer is the most common female cancer in Latvia and all over the world. The main method of treatment is still surgery but additional treatment is important as well since chemotherapy and radiation therapy can significantly affect distant results of the treatment. It is important that surgery does not affect the timely initiation of adjuvant therapy.
The aim of the study was to evaluate the breast reconstruction impact on the initiation of adjuvant therapy in breast cancer patients after mastectomy.
Materials and methods. A retrospective study was carried out by analyzing 100 breast cancer patients who were treated in Latvian Oncology Centre. The study group involved 47 patients who performed mastectomy and breast reconstruction followed by adjuvant therapy. The control group consisted of 53 patients which have been mastectomy followed by adjuvant therapy. Data have been collected and processed electronically by using Microsoft Excel 2007 and SPSS Statistics 20 programs.
Results. The average age of the patients involved in the study group was 42.9 ± 7.2 years and 54.7 ± 11.1 in the control group. The breast reconstruction after mastectomy was the most commonly done by young women aged 35 to 44 years (51.1%, 95%CI: 37.2; 64.7). Mainly (83%) the breast reconstruction was performed with the implant/expander (95%CI: 69.7; 91.1). Comparing the clinical stages in both groups, the difference between two groups was not found (p>0.05). Also histological difference between groups was not statistically significant (p>0.05), in 82% it was ductal carcinoma. The most common differential type of the breast cancer was low grade (49%, 95%CI: 39.0; 58.8): 44% in reconstruction and 53% in the control group. Comparing the hormone-receptor status, statistically significant difference between the groups was not found (p>0.05). Evaluating the lymphatic invasion, no difference between the groups was found (p>0.05). The average hospitalization time was 16.3 ± 7.0 days, the difference between two groups is 3.14 days and it is statistically shorter in the reconstruction group than in the control group (p<0.05). Comparing the average number of days from surgery to the adjuvant therapy it was found that there were no statistically significant difference between two groups (p=0.072), in the reconstruction group 31.1 ± 11.6 days and in the control group 35.8 ± 13.7. The adjuvant therapy was not cancelled after breast reconstruction due to complications. The chemotherapy was received by 94% of patients. During five years there were 6 dead patients, one of the reconstruction group and five from the control group. The expected 5-year survival does not significantly differ between the groups (p=0.224).
Conclusion. The breast reconstruction does not delay initiation of adjuvant therapy of breast cancer patients after mastectomy and does not worsen the results of distant treatment.
